The clinic provides a variety of health-related services to the students:

  • Evaluation of students with illness or injury: First aid and emergency care will be provided
  • Administering prescribed medications and treatments
  • Health Screenings
  • Case Management

It is IMPERATIVE that you notify the clinic of any health conditions that your child may have or if your
child may possibly require medical attention while at school. It is important because we will discuss a
plan of care for your child. This includes but is not limited to DIABETES, SEVERE ALLERGIES, SEIZURES,
CARDIAC CONDITIONS, OR ANY OTHER MEDICAL CONDITION that will require him/her to take
medication or need treatment while at school. The nurse will then require you to complete a Parent
Questionnaire regarding your child’s medical condition.

If your child has not seen a provider recently, please schedule a visit over the summer in preparation for
the new school year. Also, please make sure that the school has the current contact information in case
we need to reach you for an illness/injury or an emergency. 

Clinic Guidelines

  1. The clinic is intended for first aid and emergencies only. It is not a place to rest, relax, or take a nap.
  2. Students must obtain a pass from their teacher or AP prior to going to the clinic.
  3. No medication of any kind is available in the clinic unless it has already been provided to the RN by the parents.
  4. All medications that are provided to the clinic by the parents (including over the counter medications), must be kept in the clinic and be administered by the nurse or other designated staff member. Every medication must have a Physician Authorization in order to be accepted and dispensed. These forms are good for one school year.
  5. All medications must be transported by a parent or authorized adult.
  6. All medications that are carried by the student while in school, are required to have a Physician Authorization on file in the clinic.

IT IS IMPERATIVE THAT EVERY STUDENT KNOWS WHERE AND HOW TO REACH HIS/HER PARENT AT ALL TIMES IN THE EVENT OF ILLNESS OR A MEDICAL EMERGENCY. (According to Directive D-5.071)

Parent Guide for Medications at School

The following requirements MUST be met to keep medication and supplies in the school clinic or to have a student carry it in their book bag.

  • Physician Authorization for Student Medication/Treatment Form
    • Is obtained from and completed by your child’s physician
    • A new form is required each school year and for any changes that occur to the current order during the school year
    • Will include the child’s name, name of medication, treatment, dosage, time interval, route, adverse reactions, desired action, physician’s signature, and parent’s signature with best contact phone numbers
    • Fax form to the school at 561-338-1440 or bring to the School Nurse
    • One form per medication
  • Medication
    • Must be in original container with the appropriate prescription label to match the Physician Authorization
    • When filling prescriptions to be kept at school, please request the pharmacy label’s expiration date to state, “See Manufacturer’s Medication Expiration Date.”
    • Ask pharmacist to label all over the counter medications with a pharmacy label
    • Medication container must be unopened and not expired

No medications can be transported by the student to the nurse. An ADULT must bring ALL medications to the clinic. A medication count and co-signature for the delivery and receipt of medication will be required.
A current photo of the child needs to accompany the medication.
No medications will be administered unless all of the requirements have been satisfied. THERE WILL BE NO EXCEPTIONS.

When a student is on the campus of any school in Palm Beach County, the student may not carry on their person or take ANY prescription or over-the-counter medication. All Palm Beach County Schools are designated “DRUG FREE ZONES”.
